I recently followed these instructions and my DirecTiVo is working over Vonage. Here's the link. In order to get this to work reliably, I had to force the modem into a maximum 19.2kbits/sec. I was able to download the 3.1.1b upgrade for my HDVR2 using Vonage (it took 4 hours!).
