First step in this case is to power cycle your network to resolve any ISP signal instabilities.
Unplug power from SB6120, VDV-21 VD, and TEW-432BRP.
Unplug Coax cable from SB6120.
Wait 2-5 minutes.
Plug in Coax Cable to SB6120, Then power cable.
When modem indicates it is online, plug power into
Vonage
device.
When
Vonage
device has dial tone, plug power into Router.
If you still experience this problem try a hard reset of the
Vonage
device by holding down the power for no less than 60 seconds. regardless of messages displayed on the screen holding the button down for less than 60 seconds fails to reset the device.
If neither of these resolves the issue it is not solvable by yourself and you will need to call
Vonage
Technical Support @ 1-866-243-4357 to have them reload the firmware on your device. If after a firmware reload the problem persist you may require technical support to issue an RA for the device.

That looks like a hardware issue to me. Have you tried calling your ISP and check if it's an issue with their modem?
