Firmware Version: 5.01.04, I copied this from the Vonage RTP300-VD settings page Header. Its listed in the upper right hand corner. Is firmware controlled by Linksys? |

Router version firmware doesn't really ring a bell since Vonage refers to firmware based upon the Voice Module's version but after looking at the most current version, your's is up to date.
Most Current: rt-11.4.1-r021-5.01.04-r070215.img
11.4.1 = the voice module firmware
5.01.04 = Router's firmware |

OK, this is good, THANKS!
Back to the router IP. I never was able to get to the settings page until yesterday, so I didn't change the IP. This Vonage router has been attached to at three different Netgear routers, as I was only using the Vonage router for the phone at the time. Is it possible that one of the other routers assigned the new IP to the Vonage router? |

No because your dealing with the LAN IP. The LAN IP can only be changed by user intervention or by itself when it detects the same network on it's WAN port. The only other possibility is your RTP300 was issued to someone prior to you and they returned it. Maybe they configured it with the 192.168.16.1 address because they had another Vonage device connected to it's WAN. |
