My configuration with a PAP2 and PIX box is slightly different than anything I've seen on these pages. Most questions ask how to install PAP2 behind a PIX box. My question is how to install both behind the same router:

Modem -> Netgear Router | -> PIX | -> Polycom A/V unit (wired ethernet w/ DHCP from PIX)
Modem -> Netgear Router | -> PIX | -> PC 1 (wired ethernet w/ DHCP from PIX)
Modem -> Netgear Router | -> PAP2 w/ 2 Vonage numbers (wired ethernet w/ static IP)
Modem -> Netgear Router | -> PC 2 (wired ethernet w/ static IP)
Modem -> Netgear Router | -> PC 3 (wireless)
Modem -> Netgear Router | -> PC 4 (wireless)
Modem -> Netgear Router | -> Printer (wired ethernet w/ static IP)

With this configuration, if both the PIX box and the PAP2 are powered on, there is a conflict -- the PAP2 fails to show a connection for either phone (and consequently offers no dialtone) AND the PIX drops the connection to the VPN host after a few minutes. If either one of these devices is unplugged, there are no issues with the remaining system that is still enabled.

My Netgear router (not the PIX) has the following ports ranges forwarded Inbound:

HTTP (TCP) 80-80 => 192.15X.X.2 (PC2)
iChat_SIP (UDP) 5060-5060 => 192.15X.X.2 (PC2)
iChat_RTP (UDP) 16384-16403 => 192.15X.X.2 (PC2)
iChat_FTP (TCP/UDP) 5190-5190 => 192.15X.X.2 (PC2)
VONAGE_UDP1 (UDP) 10001-16383 => 192.15X.X.99 (PAP2)
VONAGE_UDP2 (UDP) 16404-20000 => 192.15X.X.99 (PAP2)

All devices and PCs in my network have assigned IP addresses in the 192.15X.X.0 subnet, and my router's internal address is 192.15X.X.1. My Netgear router is NOT configured for NAT though it does offer 10 DHCP addresses to wireless clients. The PIX box has an assigned IP address on the 192.15X.X.0 subnet (WAN connection) and issues DHCP addresses to the Poloycom and PC1 corresponding to the network on the other side of the VPN connection (10.X.X.X).

Are the PIX box and the PAP2 trying to use the same ports?

I believe the problems were caused by an IP address conflict. When my company's IT department sent me the pre-configured PIX box, they configured it with exactly the same IP address I'd assigned to my PAP2. (What are the odds?) When I changed the IP address of my PAP2, all problems disappeared.

yeah, it sounds like an IP address conflict as well. double check each one's IP address.
