I used to have the problemsw you describe, plus many other problems until I switched ISP's. I am now using RoadRunner and have not had any problems since I switched last year.
I have RoadRunner and experience the problem all the time beginning back in Feb of 2008.

I see no correlation b/w ISP and intermittent callerID issues.

Back when Wideopenwest was my service provider, I had MANY unexplained problems that Vonage Support could not solve (including CID problem). After a couple of years of aggravation I switched to Road Runner.
I have been a very happy Road Runner user for about a year now and have had NO problems with Vonage nor CID.
