I suggest unplugging all other devices including computers, and just connect your modem to your phone adapter. If you don't notice any trouble, it would be something caused by active applications on your PC (malware, torrent, etc.)
Does anyone in your family play X-Box Live? That eats bandwidth.
You could also run an extended ping test to find out when the problems are occurring, using freeware like PingPlotter or shareware PingPlotter 2. This'll let you know when and where you're getting bogged down.
Have you tried calling your ISP and saying that every week between x and x time, your connection slows down? They may be able to juggle you around on the node a little bit.
