I am having an intermittent problem where people cant hear me when they call me,

I have the following setup, Charter 5 meg cable DSL connection
Netgear WGR614 V4 wireless router
Vonage D-LINK VTA-VR

Your upload jitter is horrible. Try removing the WGR624. Use the VR right off the modem and see if the calls get better.
Or try putting the VR in the DMZ of the WGR624.

My understanding is that the Netgear WGR614 does not have QoS which can give highest priority to your dlink. As a result, if you are running any internet intensive programs, it could very easily result in bad phone quality. Even regular internet use would probably affect the call quality.

When I am on the phone, my simple internet requests take a very long time b/c my rt31p2 is delaying my computer request until there is a small gap in my phone conversation, at which time, I see it quickly get my computer request through. It's the upload that is the bottleneck b/c your download has plenty of bandwidth.

You might consider either of 2 steps.

1. Lower the bandwidth and try not to get on the computer too much or run any intensive programs while you are on the phone.
2. Get a good quality router that has QoS so that you can prioritize your Vonage dlink.

See if taking step 1 would remedy the situation enough so that you would not have to do step 2. In many ways, I wish Vonage would only sell the dlink vta to those with high quality routers b/c it's really a disservice to the customer who doesn't have a router with qos.

Also, your round trip time to the server is awful. Try repeating the test with the router removed.
