I am sitting in Alamos, Sonora, Mexico with Telmex DSL that is currently has a download of 219 and upload of 85 and a RTP300 router. My voice quality is about as good as it gets right now and that's pretty good. During the weekday, it's lousy after about 9 am when, I'm guessing, there is congestion of some sort. Today, it's Saturday and my voice quality is pretty good. Unfortunately, I had hoped to use Vonage for business calls during the week but that appears to not be workable. Have changed settings, configuration, etc., etc., and the problem seems to persist so I'm blaming it on congestion, lines, and Telmex in general. If someone reads this and has any suggestions or comments regarding service from Mexico to USA, please let me know. Thanks.

I think this article can explain some of the problems.
Sorry, it is in spanish from magazine Information Week Mexico. It discusses some dirty practices by Telmex.

Although not a fix, the best we can do is change the voice quality setting to Normal and their packet size to .30. TelMex is intentionally de-prioritizing VoIP RTP packets and need to complain to TelMex about this and/or find another ISP. Cable or Satellite ISP's may be a good choice.

Change the upload/download speed of your service.....try the second plan of telmex
