I have a relative that just signed on to OptimumVoice. Just happened this week, so I don't have much information or feedback on it. I know people at work that are using it and I haven't heard of issues thus far except when cable service interruptions happen as in the past snow storm.

(the instructions are for the WRT54GL, but the basic QoS setup is essentially the same for the WRT54G, WRT54GS, and WRT54GL routers, running both stock firmware and the aftermarket stuff I use).
I have instructions there for de-optimizing BitTorrent so you can do normal surfing, too. You can ignore those parts and just get QoS tuned and get the MAC address of the
Vonage
router set to HIGHEST.
QoS seems to be a lot more effective, in my experience, if you turn flow control off on the Ethernet Port Priority fields. Not entirely sure why, but you can try that as well, and maybe it'll help you out.
Thanks for the information. Your write-up is very good and helpful. I am actually using the QoS setup on my WRT54G router with the MAC address of my
Vonage
ATA(WRTP54G) set to the highest priority. I actually have the upstream bandwidth set to AUTO, but will be looking to manually set it after a few upload speed tests. I sure would like to find documentation on the calculation used by the WRT54G when upstream bandwidth is set to AUTO. I was thinking that it would dynamically measure the upload bandwidth via some kind of internal polling.

Well, I wanted to report that today my service seems back to normal. My ISP throughput is close to the normal performance. My only guess at the cause is the work going on with the ISP, OptimumOnline. I have a relative using OptimumOnline with another
Voip
service, and they are having the problems that I was experiencing. Looks like OptimumOnline is working through sections of their territory with changes. I have also been checking out forums on broadbandreports.com, and I see many other OptimumOnline clients having network performance problems.
