You are correct. While some older devices do require 5061 to be actually forwarded, newer ones do not. Vonage's web site defines these older ones as "devices provisioned before 2005".

So the list of ports is sort of a "catchall" if your router/firewall is not acting "normally", or if you have an older device that needs the ports forwarded, to forward any and all ports your Vonage device could possibly want. It's easier than having a list specific to each device. Vonage's web page is a little clearer on the subject:

DMZ is the "lazy man's port forwarding", or a fallback if your port forwarding isn't functioning properly. Nothing wrong with it, of course, but please do make sure if you put the Vonage device in the DMZ that you DO NOT have "remote administration" turned on on the Vonage unit. Otherwise, someone could easily type http://your_public_IP_address:8080 and get a signon screen to your Vonage device. That will force you to plug a computer into the Vonage device to administer it, of course, but if you're just using it as a Vonage device you shouldn't have to log into it very often.

For any "normal" router/firewall, and with "newer" units like the RTP300 and WRTP54G, no port forwarding should required at all. The Vonage device makes an outbound connection and establishes a server connection to Vonage's servers. At that point, any properly-operating firewall, SPI or not, should allow communications on that port as long as the Vonage device keeps the port open. Once the outbound service request has been made, the Stateful Packet Inspection should allow that request to be granted by the remote.

In this case, the WRT54GX4 SPI firewall seems to have a defect in that it is not properly respecting service requests from inside the firewall, so users of this router will have to bypass the firewall by DMZ'ing, forwarding ports, or disabling the firewall. Forwarding specific ports is probably the safest, but it's still drilling a hole in the firewall that should not have to be there. It is what it is, however, and unless Linksys fixes this defect in a new firmware release of the unit, that's the way it is.
