Vonage like many ISP has peering agreement to forward data traffic in this case Voip to POP sites around the globe. Once the data reach the nearest designated country, there are more than one peering agreements with CLEC type of companies (local to India) that connects your call to the PSTN there.
The problem is that these companies that provide the gateway to the PSTN network tries to multiplex too many calls and purposely reduce the voice quality as a result. Vonage uses the same companies in India and other countries that serve as PSTN end points for other Voip services and Calling cards. Vonage is trying to have more peering agreements with several different companies to improve the call quality. (But it's a gamble). Ah the benefit of early adopters.
