In the old days, the Local Exchange Routing Guide (LERG), was the document that dictated all of the rate centers that defined what was local and long distance calling between HNPA/NXXs. It did not matter which phone company or companies were servicing that territory. Whomever shared the rate center resulted in local calling for everyone.
Once the 1996 Telecommunications Act came into play, things really became a quagmire of how to hand off calls between the BOCs, ILECs, CLECs, etc., so the requirements for reciprocal agreements came into play. These agreements are in place to ensure that the appropriate cost settlements and shared facilities are covered between the given telcos.
Just because a phone company decides to show up to town, it does not automatically mean that the calls are to automagically show up for free on someone's doorstep without the interested parties sharing the cost of paving the street.
The end result is this: An agreement between telcos in a shared rate center = a local call. All calls without the agreements are defaulted to the nearest toll tandem, and therefore is a toll call.
