The PAP2 and RT31P2 initiate an outgoing "heartbeat" UDP packet to the Vonage servers about every 8 seconds. I don't have my router in front of me so I cannot give you a more precise time period. This UDP packet essentially says "I am here and this is who I am in case you need to get ahold of me". This establishes a connection with the Vonage servers. It is this connection that allows most of us to never have to bother with opening Vonage related ports on our local routers. Our router lets the UDP packet out and if Vonage needs to send us a phone call then they are actually responding to our last packet out which our router is typically happy to let in.

Of course your mileage can vary. Your router/firewall may be setup by default to not let certain kinds of packets in the front door no matter what. In these cases you do need to take action to open your router/firewall up a bit.
