You could overload a network and all the priority in the world won't guarantee a connection, similarly, as is the case for most people, no prioritization is required to have a reliable connection. People doing file sharing and gaming may have need to control the priority of packets leaving their network if they are occasionally saturating their link. That prioritization won't help if the ISP is doing something poorly on their end.
In my case I am using an old Motorola adapter behind an old netgear router with no QoS. My service is as reliable as POTS with the exception of some system wide outages a few months back. While I could run the Motorola between my modem and router to guarantee QoS I moved it inside because of problems with the firmware over a year ago.
Unfortunately the only way to achieve equal reliability to the PSTN would be to create a dedicated network for Voip and size it for the maximum required bandwidth. It would still be cheaper to operate than the PSTN in the long run, but it would not have the cost savings that are obtained by using the Internet. Plus everyone would have to have two connections, one for phone and one for data. This is in fact how some Voip services from the cable companies are now implemented and they cost as much as POTS.
There is a company that has built a private network to connect Voip companies directly. This allows the Voip companies to route calls to each other without having them cross the Internet between them. It does not address the customer's Internet connection however, it is mainly to reduce bandwidth costs.
