Sure, that looks like it would work. My concern was that stray voltage coming in from the POTS side could potentially get to the VoIP side, which would not be good. It looks like it's separated enough to avoid that. On the other hand, if your old provider offers call forwarding, why not just set that up instead to your new Vonage number. It's not exactly seamless, but it would seem like it, and I'm sure your monthly bill would reflect it?
