Vonage Steps that can help in fixing the one way is first to eliminate the WRT54G router

Steps that can help in fixing the one way is first to eliminate the WRT54G router from setup, Conn the Vonage adapter directly to Modem & see if the audio is fine. If still doing same then contact VOANGE technical support or u can login to your webaccount & go to feature -> bandwidth saver & bring that to Medium or Normal.

If direct conn is fine & happens with WRT54G. Try these steps :
1) Login to WRT54G webinterface -> Go to Security tab -> If there is a Seperate option Firewall -> enable or disable then choose disable ( this is how SPI firewall is disabled in Linksys routers ). But I am not sure the Version of WRT54G you have. If u dont fine this option then u can ignore it.

2) Than trying to forward all port by Portforwarding. Go to Application & Gaming tab -> go t0 DMZ sub-tab & setup the ip address of Vonage adapter in DMZ - This will have all ports open.
Most of the time. Setting up a STATIC IP in Vonage ADAPTER. Outside the N/W range & setting that up in DMZ is one of the best step.

3) This is one Very good option which fix most issues behind Routers. Check the Version Number of your linksys ( Found in the bottom eg: WRT54G v3 ), goto linksys.com & go to support & download the latest firmware. See if the Linksys router u have the latest firmware. If not , contact LINKSYS Tech support to have it upgraded. IF WRT54G has old firmware. Upgrading to latest one is the best fix.
