Regarding MAC address, you're correct about provider locking me into only one. But I have cloned the MAC address to the D-Link as well as the phone adapter. Currently, I've scrapped the D-Link router out of the equation, and just have my cable modem connected to phone adapter (using PC MAC address). Like I mentioned previously, I can connect to Internet I just can't connect up to Vonage servers. Unless I've missed some suggestions, I have already reset phone adapter numerous times, reset cable modem, released/renewed IP address, checked settings/config with cable modem tech support, opened support tkt w/ Vonage, checked actual phone(s) with POTS, etc.. If there is a possible issue with the hardware, i.e. Linksys, how would I be able to tell that?

There are several ways you can accomplish this. I just placed my phone Adapter
I have the same type of setup. Completely power off your cable modem and both routers.

Some cable companies lock you into one MAC address. In your case, that of your PC
I have a similar problem - I have a Linksys Wireless router connected to a Cox Cable
log into router. under setup, you will see a row of other options. Look for MAC address
I had one heck of a time when I added the Linksys router I got from Vonage to my D-Link
