Looking over your post there is one thing that sticks out with an intermittent issue that is constant when calling voice mail. There is one thing that I can think of that would be different about every call to voice mail and intermittent outbound calls. That difference is the codec. Vonages voice mail servers will force a connection using the G711u codec(high on your bandwidth saver settings) and some calls, especially to PSTN, might also force your connection to G711u. Seeing as how you have low bandwidth to start with this increase may be enough to kill your connection momentarily, causing a loss of dial tone/dropped call.

This seems to me that it could just be a case of not enough bandwidth to support Vonage. Vonage requires at least 90kbps at the G711u codec and that is not accounting for overhead or any use of the Internet from your computers. Assuming you currently have the Zoom 100 package, I might look into upgrading to the Zoom 500 package.
