I was having almonst an identical problem. From another forum user, it was recommended I put a "real" router in front of the WRTP54G. This doesn't have to be an expensive piece of eqipment (I used the Linksys WRT54G for $39.99 after rebates).
I have the Vonage WRTP54G connected off port 1 of my new router (goind directly into the WAN/Internet port of the WRTP54G). I disabled EVERYTHING I could find on the Vonage router (firewall, wireless, etc.). I then setup the QoS on the new router for Port 1 to have "High" priority. It has only been a few days, but I haven't "lost" a call yet. I still get a little choppiness if I do something like a speed test during a conversation, so I need to read up some more on tuning the QoS stuff (i.e. set the upload speed to a real number instead of auto, probably create the QoS setting on the device by MAC address instead of on the port, etc.)..
I know this seems to make the WRTP54G useless, as I bought it to consolidate down to 1 box. But adding a new router (which is better than the old D-Link one I used to have) for $40 seemed to be what I had to do to "fix" the problem of lost audio.
