Hi,
I had the same problem, and I'm on dsl 256k, I have my bandwidth at 50, I had called Vonage and talked to a very nice tier 2 Tech, who changed some settings on his end with packet size, jitter buffer, etc, which was a big help, but didn't completely solve my problem, they ended up sending out a new router, and I still had the same problem with a 1 Second delay.

So I ended up contacting the company who makes my modem, they got into the settings by inputting an IP address in the browser bar, that accessed the modem menu, and they made sure I had the latest firmware, and noticed that my ISP was using Interleave on my connection.

I did some research on Interleave, in case you don't know- Interleave protocol means that when a packet is sent through the internet it gets disassembled up into tiny packets, and then reassembled at the other end, this increases latency, or delay, it is commonly used as default to make sure everybody can use DSl because it is line noise resistant. There is another protocol called Fast, it isn't as tolerant to line noise, but the packets don't get disassembled, and the overall latency decreases.

I called my ISP and spoke to a level 3 Tech who changed my protocol to Fast, the delay has decreased considerably, and I went from pinging at 300ms to pinging at 100ms or less by pinging I mean going to "start" "run" and typing in "cmd" and then at the prompt type "ping yahoo.com" and then looking at the average time, since they changed me to fast protocol, webpages load faster, e-mail comes faster, and all-in-all I've had no problems.

So I would suggest first changing the bandwidth setting like Rebus said, and if that doesn't work then try calling the manufacturer of your modem, and checking the firmware version, and checking the diagnostics to see if it is receiving an interleaved signal and then call your ISp to change to a fast protocol, or if you can't reach the manufacturer to know if it's interleaved in the diagnoses of the Modem, call your ISP, talk to a level 3 tech, or System Administrator (be sure to get their name and number, in case you have any more problems), and ask them if they are using Interleaved protocol on your connection, if they are ask them to switch it to fast protocol, and give that a try.

My ISP tried to tell me it would have to be a system wide change, but I told them that I knew it could be changed at the local port, so if they tell you that it's system wide, just tell them you know it's at your local port.
