First, to understand what the cause may be, it is important to understand what generates echo. Echo is the reflection one someone's outgoing audio back to them. There are several possible causes of echo but they are always in the analog side. In order words, this would be the telephone set, wiring, analog-to digital fuctionality of your telephone adapter, the equilivalent equipment at the other end.

Echo will always be present. Most telephone sets will actually echo what you say into the microphone back into the earpiece. And, of course, you can hear what you are saying. But this type of echo is not noticable, because you are subconsciously expecting it. When it is echo that you notice, it is one with a significant delay (usually greater than 200 milliseconds). Due to this, the side that notices an echo is typically not the side that causes it.

Most telephone echo is extremely simple: the audio coming out of the ear piece is picked up by the microphone and reflected back. If this is the case, usually asking the other person you are talking with to turn down their volume will solve the problem. Other causes are long, unshielded telephone cords or damaged telephone cords where the send and return "bleeds" into each other. Historiclly, longer distance telephone calls (and especially international ones) were calls more susceptable to echo because the greater the distance, the longer it took to get the audio to the other end and back, increasing the chance that the echo (which is almost always present) is noticed.

Now in your case, the test results suggest that you may have higher than normal latency. The maximum delay value is of some concern. Similar to a long distance call, the longer that it takes for your data to transverse your internet connection, the higher the likelyhood you would notice echo. It may be helpful to perform the test again or a similar one hosted on a different site in order to compare results.
