Standard 2-Pair Telecom Color Codes (RJ-11)
PAIR 1
Tip - Green
Ring - Red

PAIR 2
Tip - Black
Ring - Yellow

Standard 4-Pair Telecom Color Codes (RJ-45)
PAIR 1
Tip - White/Blue
Ring - Blue

PAIR 2
Tip - White/Orange
Ring - Orange

PAIR 3
Tip - White/Green
Ring - Green

PAIR 4
Tip - White/Brown or White/Slate or White/Violet
Ring - Brown or Slate or Violet

Hold an RJ-45 connector tab down, copper connectors away from you (cable towards you). Pin 1 is on the left and pin 8 is on the right.

Hold an RJ-11 connector tab down, copper connectors away from you (cable towards you). Pin 1 is on the left and pin 4 is on the right.

The pins on the RJ-45 are as follows.
Pin 1 - White/Green
Pin 2 - Green
Pin 3 - White/Orange
Pin 4 - Blue
Pin 5 - White/Blue
Pin 6 - Orange
Pin 7 - White/Brown
Pin 8 - Brown

The Pins on the RJ-11 are as follows:
Pin 1 - Black
Pin 2 - Red
Pin 3 - Green
Pin 4 - Yellow

Connect pins 4&5 from the RJ45 to pins 2&3 on a RJ11 plug to connect to your Line 1 on your Voip router.
Connect pins 3&6 from the RJ45 to pins 1&4 on a RJ11 plug to connect to your Line 2 on your Voip router.

To summarize:
Line 1:
white with blue = green = tip
blue = red = ring

Line 2:
white with orange = black = tip
orange = yellow = ring
