Actually, he isn't breaking the TOS at all. The TOS is very general and lacks anything specific. Inact there is NOTHING in the TOS at all that says you can't use another SIP device, just than you agree that Vonage can terminate your account if they want.

Moreover, if he does provision his adapter himself and changes the settings by himself, that's his right. And as long as what he does doesn't hurt vonage's SIP servers, employees, bank account, or other customers, I see no reason why his account should be terminated. And even if Vonage did discover that he provisions his adapter himself, they should leave him alone. He's not breaking the TOS because he's using his own PAP2 adapter. And although Vonage prohibit's "tampering" of the adapter, that's very broad. If he tampered with the adapter yet does not hurt Vonage at all or use his service in a way that he's not paying for, then Vonage should be glad he's still a paying customer.

After all he has lots of options if Vonage chooses to cancel his account. His adapter is his, and he can unlock it and use it with another provider.

From what he seems to me, he is a paying customer that likes to have more control over devices he purchases. I see nothing wrong with that. He's honestly paying for his service, and if he wants to be responsible for provisioning the adapter himself then he should be able too, as long as he is not causing harm to Vonage. And provisioning his adapter himself isn't causing harm to Vonage to breaking the very general, very broad TOS.
