Basically I agree with NavyDavy. However, we need to look at which POTS line you're replacing. Typically, the primary (first) line is on the red-green pair and the secondary line is on the yellow-black pair. The Vonage adapter is wired so that running a cord from the adapter to a wall jack will connect you to the red-green pair, if your house wiring is standard (and you need to be sure of this).
If this is the line you're disconnecting, then everything is simple and you just plug the adapter into a wall jack after disconnecting the POTS line. If you're replacing the POTS line on the yellow-black pair (line 2) then things are more complicated. You can still connect Vonage to the house wiring by plugging into a wall jack, but you'll have to modify a cord to do it or use a suitable adapter.
If you want to wire it at the interface box as you originally indicated, you can cut the end off a four wire cord and use the red-green pair. If you're replacing the POTS line on the existing red-green pair, disconnect the POTS and connect red to red and green to green. If you're replacing the POTS line on the yellow-black pair, disconnect the POTS and connect the red wire on your cord to the yellow on the terminal block, and green to black.
