Actually, you don't even need to log into the device. From any computer connected to the WRTP54G, just go to http://192.168.15.1 and the firmware version is on the upper right hand corner of the login screen.
As Darrel correctly pointed out, 5.01.04 is the latest, 1.00.62 is the next-to-latest. Either should work just fine.
Dec 2007 was, I think, around the period when 5.01.04 came out. So that may be your problem, and you can email Vonage and ask them to revert it back to 1.00.62. Fair warning, though - I did this (because I wanted to be on 1.00.62 for other reasons) and almost immediately got static on the line that just became unbearable. I tried to fix it for several weeks, then gave up and asked them to move it back to 5.01.04.
I also do not use my Vonage WRTP54G as a primary router. It's just not nearly robust enough, and the QoS is a joke ( http://vonage.nmhoy.net/qos.html ).
Having said that, the WRTP54G is a most excellent Vonage adapter, as long as you only use it for that. (grin)
